About the position

The LPN (Licensed Practical Nurse) is responsible for providing direct nursing care to residents and supervising the daily activities of nursing assistants. This role ensures compliance with federal, state, and local regulations while maintaining high-quality care standards as directed by the Director of Nursing.

Responsibilities

  • Direct the day-to-day functions of the nursing assistants.
  • Meet with assigned nursing staff and support personnel to plan services, programs, and activities for the shift.
  • Make written and oral reports/recommendations concerning shift activities as required.
  • Admit, transfer, and discharge residents as required.
  • Provide leadership to nursing personnel assigned to the unit/shift.
  • Complete and file required record-keeping forms/charts upon resident's admission, transfer, and/or discharge.
  • Receive and transcribe telephone orders from physicians and record on the Physician's Order Form.
  • Chart nurse's notes in an informative and descriptive manner reflecting the care provided and the resident's response.
  • Fill out and complete accident/incident reports and submit to the Director as required.
  • Perform routine charting duties in accordance with established policies and procedures.
  • Prepare and administer medications as ordered by the physician.
  • Develop and maintain a good working rapport with inter-departmental personnel and other departments within the facility.
  • Create and maintain an atmosphere of warmth and positive emphasis throughout the unit and shift.
  • Receive/give nursing report upon reporting in and ending shift duty hours.
  • Make rounds with physicians as necessary.
  • Administer professional services such as catheterization, tube feedings, suction, and care for the dead/dying as required.
  • Monitor assigned personnel to ensure compliance with safety regulations in the use of equipment and supplies.
  • Maintain confidentiality of all resident care information and report unauthorized disclosures.
  • Ensure nursing care is provided in privacy and that personnel knock before entering resident rooms.
  • Perform other duties as assigned.

Requirements

  • Licensed Practical Nurse with current unencumbered state license.
  • Ability to read, write, speak, and understand the English language.
  • Ability to make independent decisions when circumstances warrant such action.
  • Knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines pertaining to nursing care facilities.
  • Current CPR Certification is required.
  • Experience in Long Term Care is preferred.
