About the position

The NP Home Visits position at Landmark, part of the Optum family, involves providing home-based medical care to chronically ill patients in the San Fernando Valley, CA. This role is integral to transforming healthcare delivery for medically vulnerable members, focusing on quality patient outcomes and satisfaction. The position requires collaboration with interdisciplinary teams to manage complex patient panels and develop care plans, ensuring compliance with state laws and long-term care policies.

Responsibilities

  • Practices in accordance with state laws and regulations governing advanced practice nursing.
  • Establishes and maintains effective relationships with members, their representatives, and facility staff.
  • Performs initial comprehensive assessments on newly enrolled members and updates care plans as needed.
  • Completes follow-up and post-hospital assessments according to standard operating procedures.
  • Prescribes appropriate diagnostics and interventions to avoid unnecessary transfers or acute admissions.
  • Consults with hospital physicians following member transfers.
  • Collaborates with UM staff and PCPs to enable medical necessity determinations for requested services.
  • Educates members and their representatives on disease processes and wellness management.
  • Supports facility staff in developing and providing educational programs.
  • Participates in management meetings and quality improvement initiatives.
  • Keeps current on relevant medical and nursing research and technology.

Requirements

  • Completion of Nurse Practitioner or Physician's Assistant accredited program.
  • Current, unrestricted NP or PA Licensure in State of Practice.
  • NP must actively hold, unrestricted RN licensure in State of Practice.
  • Certification through AANP, ANCC, or NCCPA.
  • 2+ years of work experience in long-term care and community resources.
  • Experience with CPT, ICD-9, and HCPC coding.
  • Clinical standards of care experience.

Nice-to-haves

  • 2+ years clinical experience in a similar role.
  • Awareness of UM standards, NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts.
