About the position

The NP or PA position at Optum Home & Community Care involves providing home-based medical care to patients, particularly focusing on managing chronic illnesses in a complex elderly population. The role emphasizes preventive care, patient education, and collaboration with a multidisciplinary clinical team to enhance patient outcomes and satisfaction. This full-time opportunity requires the provider to engage with patients in their homes, ensuring comprehensive assessments and timely documentation while avoiding unnecessary hospitalizations.

Responsibilities

  • Manage chronic illnesses in a complex elderly population with multimorbid conditions.
  • Document timely with a focus on closing HEDIS quality gap measures and recapturing HCCs.
  • Engage new patients with a comprehensive geriatric-focused initial assessment.
  • Triage, diagnose, and treat patients on an urgent basis in home settings.
  • Order and interpret appropriate tests including x-rays, labs, and EKGs.
  • Start treatment in the home at the point of care.
  • Collaborate with the patient's PCP and specialists as needed.

Requirements

  • Completion of Nurse Practitioner / Physician Assistant accredited program.
  • Current state NP / PA license.
  • Knowledge of CPT, ICD-9 and HCPC codes.
  • Proven knowledge of clinical standards of care.
  • Proven knowledge of Windows 2000 and MS Office Word Processing software.
  • Ability to read, analyze, and interpret general business periodicals and professional journals.
  • Ability to write reports, business correspondence, and procedure manuals.
  • Ability to effectively present information and respond to questions from groups.
  • Proven ability to perform basic mathematical operations and solve practical problems.
  • Access to reliable transportation.

Nice-to-haves

  • 2+ years of clinical experience.
  • Knowledge of long-term care and community resources.
  • Awareness of UM standards, NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts.
