About the position

The Nurse Practitioner and Physician Assistant will provide patient-centered care in patients' homes or facilities, focusing on delivering high-quality medical services while ensuring accurate documentation and compliance with company metrics. This role involves performing initial screenings, managing urgent care needs, and prescribing appropriate treatments, including palliative and end-of-life care.

Responsibilities

  • Conduct initial patient screenings and physical examinations.
  • Create assessment plans for patient care.
  • Address urgent care needs of patients as required.
  • Order, interpret, and analyze diagnostic tests.
  • Determine the need for follow-up appointments and further treatment options.
  • Prescribe and manage medications and treatments, including psychopharmacological agents.
  • Deliver palliative care and end-of-life care to patients and families.
  • Maintain accurate and current patient records.
  • Schedule initial, urgent, and follow-up appointments as needed.
  • Ensure compliance with company metrics related to documentation, coding, and patient engagement.

Requirements

  • Experience in family practice, geriatrics, hospice care, palliative care, hospitalist, ICU, SICU, or ER.
  • Minimum Master's Degree in a relevant field.
  • Valid nurse practitioner or physician assistant license and Board Certification.
  • NPI (National Provider Identifier).
  • BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certification.
  • Valid driver's license and auto insurance.
  • Willingness to drive to patients' homes or locations.

Benefits

  • Salary range of $103,000 - $137,000 depending on experience.
