About the position
The Nurse Practitioner (NP) at CenterWell Senior Primary Care is responsible for applying advanced education and clinical competencies to achieve optimal patient outcomes. This role involves diagnosing and treating acute illnesses and chronic conditions, prescribing medication, and providing health promotion and maintenance. The NP works independently, exercising judgment on complex issues while contributing to the Tuberculosis (TB) screening program. The position emphasizes a commitment to improving consumer experiences and delivering personalized care to seniors.
Responsibilities
- Diagnose and treat acute illnesses and chronic conditions.
- Prescribe medication and provide treatment plans.
- Conduct health promotion and maintenance activities.
- Exercise independent judgment on complex clinical issues.
- Advise on functional strategies related to patient care.
- Participate in the Tuberculosis (TB) screening program.
Requirements
- Bachelor's degree in a relevant field.
- 8 or more years of technical experience.
- 2 or more years of project leadership experience.
- Active, unrestricted Nurse Practitioner/PA license in the appropriate state.
