Responsibilities

• Provides comprehensive nursing care and care management services to patients based on nursing assessments and physician's medical plans.
• Participates as a crucial member and leads the care team.
• Utilizes the nursing process to identify patient problems and develop, implement, and evaluate care plans.
• Communicates patient needs and recommended treatments to the care team.
• Triages patients who call or walk in and directs them appropriately.
• Provides patient education and self-management support through regular follow-up calls.

Requirements

• A graduate or higher-level degree in nursing from an accredited program is required.
• Must possess a current, active, full, and unrestricted license as a professional nurse.
• At least one full year of professional nursing experience equivalent to the GS-9 grade level.

Nice-to-haves

• Experience in a specialty area of nursing such as Acute Care, Community Health, or Family Nursing.
