About the position

The position involves providing clinical and administrative support in a healthcare setting, ensuring the safe and efficient operation of the clinic. The role requires making independent clinical decisions, performing case management functions, and coordinating care with physicians. The candidate will be responsible for patient education, documentation, and fostering a collaborative environment among care team members. The position also emphasizes the importance of professional development and participation in quality improvement efforts.

Responsibilities

  • Demonstrates appropriate judgment skills to make independent clinical decisions in routine patient care matters
  • Ensures the safe, productive, and efficient operation of the clinic through timely and attentive administrative and clinical support
  • Demonstrates skills in phlebotomy, obtaining EKG's, injectable medications including immunizations, and administering IM antibiotics
  • Performs case management functions; coordinating with internal and external resources to support patient
  • Demonstrates prioritization of patient needs and patient flow, phone triage, provider support, nursing assessment, patient education, patient scheduling, and referrals
  • Formulates goals and an individualized plan of care for at-risk patients that involves the patient as partner and considers the individual needs of the patient in a holistic manner; evaluate and revise as necessary
  • Triages patient situations and coordinates care with physician. Assist physician with all aspects of patient care including assessment, evaluation and education
  • Demonstrates critical thinking in the identification of clinical, social, safety, psychological, spiritual and environmental needs for the patient care within an episode of care
  • Practices solid communication skills, and is able to articulate and translate the patient's condition to other care providers and to negotiate and make recommendations for changes in patient care and office practices
  • Patient documentation is comprehensive and promotes communication between caregivers
  • Fosters an environment for open communication and collaborative practice, communicates effectively in oral and written formats
  • Demonstrates accountability for professional development that improves the quality of professional practice and the quality of patient care
  • Actively participates in practice-based shared governance, goal setting and supports the change and transition process to improve quality of care and the practice environment
  • Serves as a highly engaged and full partner on the care team and responds willingly to care team member needs for assistance and partnership
  • Participates in work that improves patient care and the professional practice environment
  • Actively participates in department or practice-specific quality improvement efforts
  • Takes personal responsibility in improving patient satisfaction with the quality of care and service
  • Utilizes research and evidence-based practice to support improvement in clinical care
  • Demonstrates knowledge of adult learning principles and applies in teaching of patients, families, students and new staff
  • Creates an environment of open dialogue, inquiry and continuous development by asking for feedback and improving practice
  • Performs other duties, as assigned by manager
  • Prolonged sitting and/or standing and moving about throughout the workday
  • Occasional local travel between clinic sites
  • Daily exposure to potentially biohazardous materials

Requirements

  • Current Pennsylvania RN licensure required
  • Current CPR certification child and adult, BLS required
  • Experience in outpatient and/or community setting preferred
  • Experience with pediatrics and adults highly desirable
  • Act 33 with renewal
  • Act 34 with renewal
  • Act 73 FBI Clearance with renewal
  • Annual influenza vaccine
  • Must obtain or provide proof of (within 14 days of hire): PPD, Tdap, Hepatitis B Immunity, MMR Immunity, Varicella Immunity
  • Complete self-attestation of fitness for duty within 14 days of hire

Nice-to-haves

  • Demonstrated success in customer service/patient, preferably in a medical care facility
  • Demonstrated success in managing difficult customer situations
  • Strong computer skills including Outlook, Word, Excel and PowerPoint preferred
  • Significant knowledge of medical practices and procedures in a primary care environment
  • Attends a minimum of one outreach event per calendar year
