The RN Case Manager in Care Transitions at Beth Israel Deaconess Medical Center plays a crucial role in facilitating the progression of care for hospitalized patients. This position involves collaborating with an interdisciplinary care team to ensure efficient, high-quality care delivery, focusing on safe and timely patient discharges. The RN Case Manager is responsible for navigating health system services, advising on compliance, and driving metrics related to throughput, clinical effectiveness, and fiscal responsibility.

Responsibilities

• Collaborate with the healthcare team to develop the plan of care and patient flow.
• Review all cases within 24 - 48 hours of admission and daily throughout the stay to facilitate care progression.
• Complete initial assessments of all admissions to identify barriers impacting length of stay and discharge planning.
• Navigate the care delivery system, ensuring timely tests, treatments, and procedures are performed.
• Articulate and communicate the plan of care to care team members and patients/caregivers.
• Create and coordinate the overall transition plan of care based on assessments and collaboration with various stakeholders.
• Participate in daily multidisciplinary rounds and communicate the plan to the healthcare team.
• Identify clinical guidelines to establish anticipated lengths of stay and appropriate patient status.
• Proactively interface with payers to verify coverage for anticipated discharge needs.
• Document assessments and care plans in a timely manner, ensuring compliance with department policy.

Requirements

• RN licensure in the state of Massachusetts
• 3-5 years of experience in an acute care setting
• BLS certification required
• ACM, CCM, or CMAC certification preferred
• Bachelor's degree in nursing or a healthcare-related field preferred
