About the position

The RN Case Manager at Beth Israel Deaconess Medical Center plays a crucial role in the Triad Model of Care Transitions, collaborating with an interdisciplinary care team to ensure efficient and high-quality care for hospitalized patients. This position focuses on facilitating safe and timely discharges, navigating health system services, and ensuring compliance with healthcare industry standards. The RN Case Manager is responsible for driving throughput metrics, clinical effectiveness, and fiscal responsibility while supporting patients through their care transitions.

Responsibilities

  • Collaborate with the healthcare team to develop the plan of care and patient flow.
  • Review all cases within 24 - 48 hours of admission to facilitate care progression.
  • Complete initial assessments of all admissions to identify barriers impacting length of stay and discharge planning.
  • Navigate the care delivery system to ensure timely tests, treatments, and consults are performed.
  • Articulate and communicate the plan of care to care team members and patients/caregivers.
  • Create and coordinate the overall transition plan of care based on assessments and collaboration with various stakeholders.
  • Participate in daily multidisciplinary rounds and communicate the plan to the healthcare team.
  • Identify and communicate anticipated length of stay and care progression barriers to the team.
  • Facilitate smooth care transitions by arranging clinical follow-ups and referrals to post-acute providers.
  • Document case management assessments and care plans in a timely manner.
  • Communicate problems affecting patient flow, satisfaction, and outcomes to the department director.
  • Maintain knowledge of organizational policies and regulatory requirements for clinical care and discharge planning.

Requirements

  • RN licensure in the state of Massachusetts
  • BLS certification
  • 3-5 years of experience in an acute care setting
  • Bachelor's degree in nursing or a healthcare-related field (preferred)
  • ACM, CCM, or CMAC certification (preferred)
