About the position

The Staff Registered Nurse (Community Care Coordinator) is responsible for overseeing and providing direct patient care to Veterans, ensuring high-quality and timely services in the community. This role involves assessing, planning, implementing, and evaluating care, while serving as a liaison between Veterans and healthcare providers. The position is temporary, not remote, and requires the employee to live within 50 miles of the Vancouver, WA VA Medical Center.

Responsibilities

  • Assess, plan, implement, and evaluate the effectiveness of Veteran's care.
  • Support timely access to care for Veterans in the community.
  • Manage the care of Veterans throughout the consult process.
  • Collaborate with interdisciplinary teams to influence care outcomes.
  • Comply with VA-mandated processes and documentation requirements.
  • Foster a safe and supportive environment for healthcare professionals.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the U.S.
  • Graduation from a school of professional nursing approved by the appropriate accrediting agency.
  • Proficiency in basic written and spoken English.

Nice-to-haves

  • Bachelor of Science in Nursing (BSN) preferred.
  • Care coordination experience preferred.
  • Critical thinking and problem-solving skills preferred.
