About the position

The Staff Registered Nurse is responsible for overseeing and providing direct patient care, ensuring high-quality and timely services to Veterans. This role involves assessing, planning, implementing, and evaluating patient care, while also serving as a liaison between Veterans and healthcare providers. The position requires collaboration with interdisciplinary teams to manage care processes and improve access to services within the Veterans Health Administration.

Responsibilities

  • Assess, plan, implement, and evaluate the effectiveness of patient care for Veterans.
  • Serve as a liaison to both internal and community providers, managing the care of Veterans throughout the consult process.
  • Support the medical center's mission to improve timely access to care for Veterans seeking community services.
  • Collaborate within the Office of Integrated Veteran Care to triage Requests for Services and coordinate care processes.
  • Develop care coordination plans and communicate care coordination needs to support Veterans through their episodes of care.
  • Influence care outcomes by collaborating with interdisciplinary team members in both the VA and community settings.
  • Comply with VA-mandated processes and utilize health information solutions for care coordination.
  • Support colleagues through knowledge sharing to provide safe, quality nursing care.
  • Evaluate outcomes of evidence-based decisions and practice changes for individuals and groups.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a U.S. State, Territory, or Commonwealth.
  • Graduate of a school of professional nursing accredited by recognized accrediting bodies.
  • Proficiency in basic written and spoken English.

Nice-to-haves

  • Bachelor of Science in Nursing (BSN) preferred.
  • Care coordination experience preferred.
  • Critical thinking and problem-solving skills preferred.
