About the position

The Nurse Home Visitor position at Adams County involves providing home visitation services to families referred by Human Services. The role focuses on supporting families through nursing assessments, education, and resources to promote self-sufficiency and healthy caregiver-child interactions. The nurse establishes therapeutic relationships with clients, assesses their needs, and provides health education and case management, all while working towards program goals.

Responsibilities

  • Provide nursing assessments and interventions to families referred by Human Services.
  • Establish and maintain trusting therapeutic relationships with clients.
  • Conduct health education and resource coordination for families.
  • Monitor and support client engagement in the program.
  • Collaborate with community agencies and service providers for client referrals.
  • Complete clinical documentation and assessments in a timely manner.
  • Participate in quality improvement efforts and case load reviews.
  • Promote public health and NSP services through community engagement.

Requirements

  • Bachelor's degree in nursing from an accredited college or university.
  • Active Colorado Nursing license and current CPR certification.
  • Minimum of one year nursing experience for PHN I; three years for PHN II, preferably in Maternal Child Health.
  • Proficient in Microsoft Office tools and online meeting platforms.
  • Valid driver's license and access to dependable transportation.

Nice-to-haves

  • Experience in community-based services and public health nursing.
  • Skills in creative problem solving and critical thinking.
  • Experience in teaching, precepting, and mentoring new nurses.
