About the position

The Nurse Home Visitor position at Adams County involves providing home visitation services to families referred by Human Services. The role focuses on supporting families through nursing assessments, education, and resources to promote self-sufficiency and healthy caregiver-child interactions. The nurse establishes trusting relationships with clients, assesses their needs, and provides health education and case management, aiming to reduce further county involvement and foster proactive change.

Responsibilities

• Provide nursing assessments and interventions to families referred by Human Services.
• Establish and maintain trusting therapeutic relationships with clients throughout the program phases.
• Conduct needs assessments and case management, including direct nursing assessments and resource coordination.
• Promote health maintenance through education on prenatal and postpartum care, nutrition, mental health, and child development.
• Maintain an active case load and monitor client engagement in the program.
• Collaborate with community agencies and service providers for client referrals and support.
• Participate in community efforts to promote public health and NSP services.

Requirements

• Bachelor's degree in nursing from an accredited college or university.
• Active Colorado Nursing license and current CPR certification.
• Minimum of one year nursing experience for PHN I, preferably in Maternal Child Health.
• Minimum of three years nursing experience for PHN II, preferably in Maternal Child Health.
• Proficient in Microsoft Office tools and online meeting platforms.
