About the position

The Nurse-LPN MDS Coordinator at Genesis HealthCare plays a crucial role in the Resident Assessment Process by completing the Minimum Data Set (MDS) and accompanying Care Area Assessments (CAAs). This position ensures appropriate patient care and nursing center reimbursement while directing the interdisciplinary team in the MDS process. The coordinator is responsible for developing individualized resident care plans and training new team members on clinical content and electronic completion of MDS items.

Responsibilities

  • Direct the nursing center interdisciplinary Resident Assessment Instrument (RAI) process while following guidelines for completion, computer encoding, and electronic State transmission of the MDS.
  • Participate in developing individualized resident Care Plans, identifying the needs of the resident and projected outcomes as required by Federal and State regulations.
  • Train new members of the interdisciplinary team on clinical content and electronic completion of the applicable MDS items.

Requirements

  • Must be an LPN in good standing and currently licensed by the state.
  • Previous experience in a hospital, long-term facility, or other healthcare-related facility is required.
  • Knowledgeable of general, rehabilitative and restorative nursing and medical practices and procedures, and laws, regulations, and guidelines governing long-term care.
  • Computer skills including Windows applications are required.
  • AccuMed experience is preferred.
  • Previous experience as an MDS Coordinator is preferred.
  • Prior experience in completing MDSs and Care Plans is required.
  • Demonstrated proficiency in RAI/PPS processes is required.
  • Must maintain current BLS/CPR certificate (excluding ALFs/ILFs).

Nice-to-haves

  • Experience with AccuMed software is preferred.
