About the position

The Nurse Manager for Kidney Care Coordination at Carolina Kidney Partners is responsible for overseeing patient care coordination, ensuring optimal patient outcomes, and managing clinical staff within the organization. This role requires a strong understanding of patient care systems, effective communication, and collaboration with healthcare professionals. The manager will also be involved in data analytics to measure quality outcomes and will play a key role in onboarding new practices and maintaining relationships with existing ones.

Responsibilities

  • Coordinate patient care and ensure necessary support throughout treatment journeys.
  • Onboard new practices and maintain ongoing relationships with existing practices.
  • Supervise clinical staff and ensure adherence to quality metrics in CKP's practices.
  • Measure quality outcomes, population costs, and patient satisfaction using CMS and physician quality data.
  • Provide feedback on operational performance to administration and medical groups.
  • Oversee the CKP care coordination system and develop methods to direct patients to the correct providers.
  • Devote 50% of effort to care coordination and 50% to managerial responsibilities.
  • Contribute to the development of dialysis and transplant education products.
  • Manage metric dashboards and ensure data integrity for reporting.
  • Facilitate transitional care and end-of-life counseling referrals at all sites.

Requirements

  • Bachelor's degree required; Master's degree preferred.
  • RN licensure required and BLS certification.
  • 3-5 years of work experience in a healthcare setting, with leadership experience.
  • 2-3 years of work experience in kidney/renal disease management.
  • Strong computer and IT skills for data management and reporting.
  • Excellent organizational and time-management skills.
  • Strong communication skills, both written and verbal.
  • Demonstrated leadership skills and ability to work as part of a team.

Nice-to-haves

  • Experience with electronic health record systems.
  • Knowledge in kidney disease management.
  • Interest in gaining practical experience in patient care management.
