About the position

The Nurse Navigator is a leadership role focused on providing support, mentorship, and guidance to healthcare providers delivering direct patient care. This position is responsible for coordinating patient care delivery, ensuring timely and high-quality care, and managing both inpatient and outpatient populations. The Nurse Navigator plays a crucial role in patient discharge planning, length of stay management, and adherence to regulatory guidelines, all while promoting a seamless care model and optimizing performance metrics.

Responsibilities

  • Coordinate the delivery of patient care by assessing individual patient care requirements.
  • Ensure patients receive timely, safe, effective, and high-quality care.
  • Collaborate with bedside nurses to coordinate the patient's plan of care.
  • Set expectations regarding professional conduct for care providers.
  • Hold direct and indirect care providers accountable for their responsibilities and adherence to guidelines.
  • Manage assigned inpatient and outpatient populations.
  • Facilitate pre-op education and daily rounding for hospitalized patients.
  • Oversee patient discharge disposition and follow-up care.

Requirements

  • Bachelor of Science in Nursing (BSN) preferred.
  • Current nursing licensure in the state of employment.
  • 2-3 years of nursing experience required.
  • Certifications in BLS and ACLS within 90 days of hire/transfer.
