About the position
The Nurse Navigator at Mercy is a leadership role focused on providing support, mentorship, and guidance to direct patient care providers. This position is responsible for coordinating patient care delivery, ensuring timely and high-quality care, and managing both inpatient and outpatient populations. The Nurse Navigator plays a crucial role in facilitating a new care model, promoting safety, quality, and financial efficiency, while also overseeing discharge planning and adherence to regulatory guidelines.
Responsibilities
- Coordinate the delivery of patient care by assessing individual patient care requirements.
- Ensure patients receive timely, safe, effective, and high-quality care.
- Collaborate with bedside nurses to coordinate the patient's plan of care.
- Set expectations regarding professional conduct for care providers.
- Hold direct and indirect care providers accountable for their professional responsibilities.
- Manage the assigned inpatient and outpatient population.
- Facilitate the new care model to support quality patient care and safety.
- Coordinate patient discharge planning for post-acute services.
- Perform pre-op education and daily rounding while patients are hospitalized.
- Oversee patient discharge disposition and follow-up care.
Requirements
- Graduate from an approved school of nursing.
- Current RN (Registered Nurse) license in the state of practice.
- 2-3 years of nursing experience required.
- BLS (Basic Life Support) certification.
Nice-to-haves
- Bachelor of Science in Nursing preferred.
- ACLS (Advanced Cardiac Life Support) certification preferred.
