About the position

The Nurse Navigator at MedStar Health is responsible for coordinating patient care across the continuum, ensuring adherence to prescribed treatment plans and national guidelines. This role involves educating patients and their families, monitoring patient outcomes, and collaborating with healthcare team members to enhance patient experiences and outcomes. The Nurse Navigator serves as a clinical expert and professional role model, promoting nursing excellence and shared decision-making within a supportive environment.

Responsibilities

  • Obtain baseline information for developing individualized care plans.
  • Work with an interdisciplinary team to manage patient care plans.
  • Assist patients and families with scheduling ancillary testing and services.
  • Provide initial and ongoing education related to disease processes and treatment plans.
  • Act as a liaison between patients, families, and healthcare teams to coordinate care.
  • Advocate for patients to identify and eliminate barriers to care.
  • Streamline the referral process and ensure timely transitions into specialty services.
  • Connect patients with appropriate resources and support services for diagnosis and treatment.
  • Monitor patient progress and evaluate the effectiveness of care.
  • Collaborate with healthcare teams to analyze performance data and participate in quality improvement activities.

Requirements

  • Associate's degree in Nursing required; Bachelor's degree preferred.
  • 3-4 years of progressively responsible clinical nursing experience in a specialty area required.
  • Registered Nurse (RN) license in the District of Columbia required.
  • CPR certification (includes BLS and NRP) required within 90 days of hire.
  • Professional certification in clinical area required within one year of hire if appropriate (e.g., OCN, CCRN).
