About the position

The PACT Care Manager RN is responsible for the care of Veterans with acute and/or chronic medical, surgical, and/or mental health conditions. This role requires focused assessment, critical reasoning, and clinical judgment to provide appropriate care and treatment, especially for patients with complex or life-threatening conditions. The RN coordinates care across various outpatient settings, ensuring patient safety and quality nursing care while respecting individual needs and preferences.

Responsibilities

• Provide coordination and transition of care across the continuum in outpatient settings.
• Focus on patient safety and quality of nursing care by applying appropriate nursing interventions.
• Identify and clarify patient needs, perform procedures, and conduct health education.
• Promote patient advocacy and assist patients in navigating the healthcare system.
• Evaluate patient outcomes and facilitate continuity of care using the nursing process.
• Collaborate with multidisciplinary teams to coordinate appropriate healthcare services.

Requirements

• U.S. Citizenship.
• Current, full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the U.S.
• Graduation from a school of professional nursing approved by the appropriate accrediting agency.
• Proficiency in basic written and spoken English.
• Ability to pass a pre-employment physical examination.

Nice-to-haves

• PACT experience preferred.
• Ambulatory Care Certification preferred.
