About the position

The Patient Safety Nurse (PSN) serves as a patient safety expert within the VA Western Colorado Health Care System, reporting to the Patient Safety Manager and supported by the Chief of Quality, Safety, and Value. This role is crucial in developing, implementing, and educating on patient safety systems while fostering a culture of safety throughout the facility.

Responsibilities

  • Assist the survey preparedness team in translating VHA, regulatory, and accrediting body standards into actionable strategies.
  • Provide guidance and support to various levels of leadership during external surveys and continuous survey readiness activities.
  • Perform various degrees of rounding throughout the facility to ensure patient safety compliance.
  • Act as a point of contact for patient safety policies, activities, and education within the Western Colorado Health Care System.
  • Review Joint Patient Safety Reports (JPSRs) daily and route them to appropriate parties for action/review.
  • Coordinate and participate in Root Cause Analysis (RCA) and Healthcare Failure Mode Effects Analysis (HFMEA) teams.
  • Participate in the development, maintenance, and operation of the patient safety program at the facility level.

Requirements

  • U.S. Citizenship; non-citizens may only be appointed when it is not possible to recruit qualified citizens.
  • Graduation from a school of professional nursing approved by the appropriate accrediting agency.
  • Current, full, active, and unrestricted registration as a graduate professional nurse in a state, territory, or commonwealth of the U.S.
  • Proficiency in basic written and spoken English.

Nice-to-haves

  • Experience with Root Cause Analysis (RCAs) and Healthcare Failure Mode Effects Analysis (HFMEAs).
  • Knowledge of High Reliability Organization Principles and Just Culture Principles.
  • Familiarity with patient safety principles and process/clinical improvement principles.
