About the position

The Nurse Practitioner at Buckeye Hospice and Palliative Care plays a crucial role in providing hospice services, ensuring that patients receive compassionate and quality care. This position involves collaborating with the interdisciplinary team to evaluate hospice eligibility, manage patient care, and support both clinical and administrative staff. The Nurse Practitioner is responsible for direct medical services, documentation of care, and maintaining knowledge of current trends in hospice care and symptom management.

Responsibilities

  • Complement attending physician care and act as a medical resource person to the interdisciplinary group (IDG).
  • Perform face-to-face visits as needed and evaluate hospice eligibility in consultation with the hospice interdisciplinary team.
  • Ensure that patients receive appropriate measures to control uncomfortable symptoms.
  • Collaborate with the IDG to meet the needs of the patient.
  • Support clinical and administrative staff in providing compassionate quality care.
  • Review clinical information and coordinate patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
  • Provide direct medical services to patients in the absence of the attending physician.
  • Attend IDG conferences as needed and participate in plan of care development.
  • Manage oversight of medications and treatment, documenting care provided in the patient's clinical record.
  • Maintain current knowledge of the latest research and trends in hospice care and pain/symptom management.
  • Participate in performance improvement programs and develop treatment protocols.
  • Demonstrate knowledge in communications and counseling patients/families regarding end-of-life issues.
  • Participate in resolving interpersonal conflicts and clinical/ethical concerns.

Requirements

  • Master's degree in a relevant field.
  • At least 1 year of experience in a similar role.

  • Experience in hospice or palliative care settings.
  • Knowledge of Medicare regulations and standards.
