About the position

Optum is seeking a Nurse Practitioner to join our Home-based Medical Care team in Cumberland County, NC. This role involves providing home-based medical care to complex, chronic patients, focusing on improving their health and well-being. The Nurse Practitioner will work collaboratively with an interdisciplinary team to develop and implement patient care plans, ensuring a holistic approach to patient care that addresses physical, mental, and social needs.

Responsibilities

  • Practices in accordance with state laws and regulations governing advanced practice nursing.
  • Establishes and maintains effective relationships with patients, caregivers, and interdisciplinary team members.
  • Performs initial comprehensive assessments on newly enrolled patients and provides ongoing care.
  • Develops and updates patient management care plans as needed.
  • Meets with patients and legal representatives to review care plans, involving the PCP when applicable.
  • Completes follow-up and post-discharge assessments according to standard operating procedures.
  • Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions.
  • Performs basic nursing procedures such as IV placement, blood draws, and wound care.
  • Consults with clinical teams following patient transfers.
  • Educates patients and caregivers on disease processes and wellness management.
  • Participates in ongoing meetings related to patient care and clinical excellence.
  • Implements quality initiatives to ensure high standards of care.
  • On-call for caseload during business hours and participates in after-hours on-call program.

Requirements

  • Completion of Nurse Practitioner accredited program.
  • Current NP license.
  • Current state RN license.
  • Knowledge of long-term care and community resources for patient care delivery.
  • Access to reliable transportation.

Nice-to-haves

  • 2+ years of clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
  • Knowledge of CPT, ICD-10, and HCPC codes.
  • Awareness of NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts.
