About the position
Geisinger Medical Center Muncy is seeking an experienced Nurse Practitioner or Physician Assistant to join the Primary Care team in Muncy, PA. This role focuses on providing comprehensive care to the senior patient population, emphasizing collaboration with a multidisciplinary team to enhance patient outcomes and access to care.
Responsibilities
- Conduct forty-minute appointments for senior patients.
- Participate in daily huddles with care team members to review schedules and patient needs.
- Collaborate with pharmacists for patient education and medication management.
- Work with care managers and health associates to coordinate patient care.
- Utilize eVisits and patient portals to increase access to care.
Requirements
- Master's Degree in Physician Assistant or related field.
- Certified Physician Assistant - National Commission on Certification of Physician Assistants (NCCPA).
- Basic Life Support Certification.
Nice-to-haves
- Experience in community medicine or primary care.
- Familiarity with electronic health records (EHR).
