About the position

The Nurse Practitioner - Complex Care at Home position at Landmark, part of the Optum family, involves providing home-based medical care to chronically ill patients in Butler and Armstrong County, PA. The role focuses on managing complex patient panels, collaborating with interdisciplinary teams, and ensuring high-quality patient outcomes. The Nurse Practitioner will work closely with patients, their families, and other healthcare professionals to develop and implement care plans, while also participating in quality improvement initiatives.

Responsibilities

  • Practices in accordance with state laws and regulations governing advanced practice nursing.
  • Establishes and maintains effective relationships with members, facility staff, and collaborating physicians/providers.
  • Performs initial comprehensive assessments on newly enrolled members and develops management care plans.
  • Completes follow-up and post-hospital assessments according to standard operating procedures.
  • Prescribes appropriate diagnostics and interventions to avoid unnecessary transfers or acute admissions.
  • Consults with hospital physicians following notification of member transfer.
  • Collaborates with UM staff and PCPs to enable medical necessity determinations for requested services.
  • Educates members and their representatives on disease processes and wellness management.
  • Provides counsel and support to facility staff and participates in educational programs.
  • Participates in management meetings and quality improvement initiatives.
  • Keeps current on relevant medical and nursing research and technology.

Requirements

  • Completion of Nurse Practitioner accredited program.
  • Licensed as NP in the state of practice.
  • Knowledge of long-term care and community resources.
  • Knowledge of CPT, ICD-9, and HCPC codes.
  • Knowledge of clinical standards of care.
  • Access to reliable transportation.

Nice-to-haves

  • 2+ years of clinical experience.
  • Awareness of UM standards, NCQA requirements, CMS guidelines, and Medicaid/Medicare contracts.
  • Ability to read and interpret business periodicals and professional journals.
  • Ability to write reports and business correspondence.
  • Ability to effectively present information to groups.
  • Ability to perform basic mathematical calculations and solve practical problems.
