About the position
The Nurse Practitioner position at Rainbow Community Care is a flexible role focused on providing hospice and palliative care services in various settings, including home hospice and inpatient care. The Nurse Practitioner will collaborate with medical teams to deliver comprehensive care, perform clinical procedures, and ensure effective patient management and education.
Responsibilities
- Perform clinical care including bedside evaluations, patient histories, and physical examinations.
- Conduct follow-up inpatient care and discharge orders, providing patient instructions and education.
- Assess patient needs and anticipate service requirements prior to discharge to home.
- Coordinate with social workers, case managers, and ancillary staff regarding hospice and palliative care services.
- Order, perform, and interpret laboratory and radiology tests as appropriate.
- Prescribe medications, including controlled substances, as delegated and licensed.
- Order treatments and durable medical equipment as indicated.
- Educate clinical staff, patients, and families on disease management and prevention strategies.
- Prepare and maintain accurate patient records, charts, and documents to support hospice and palliative practices.
- Assist in the development of clinical documentation protocols.
- Demonstrate empathy and sensitivity to the needs of patients and families.
Requirements
- Current Nurse Practitioner License for Wisconsin.
- A minimum of one year's nurse practitioner experience is preferred.
- Previous Hospice and Palliative Care experience is preferred.
- Valid driver's license, reliable auto, and current auto insurance required.
- Strong teaching and leadership skills required.
- Current Cardiopulmonary Resuscitation certification.

- Clinical documentation experience preferred.
- Ability to relate to people of diverse cultures and ethnic backgrounds.
- Ability to mentor students and provide input for evaluations.
