About the position

The Nurse Practitioner (NP) position at Fallon Health's Summit ElderCare program involves providing primary health care to elderly participants across various PACE sites in Massachusetts. This role emphasizes disease prevention, wellness, and management of chronic and episodic illnesses, collaborating closely with a multi-disciplinary team. The NP will float between sites based on business needs, with some remote work possible. The position offers a predictable Monday to Friday schedule, focusing on quality care in a supportive environment.

Responsibilities

  • Serve as a float Nurse Practitioner (NP) to each of the Summit ElderCare sites throughout Massachusetts.
  • Provide primary health care to Summit ElderCare program participants focusing on disease prevention, wellness, and management of chronic and episodic illness.
  • Collaborate with the program's Primary Care Providers and multi-disciplinary team to establish realistic goals and develop individualized plans of care.
  • Perform initial and periodic assessments of participants and document problems/issues in medical evaluations.
  • Evaluate episodic illnesses and implement appropriate diagnostic and therapeutic interventions.
  • Conduct routine monitoring of chronic, stable conditions and implement plans of care effectively.
  • Participate in multi-disciplinary team meetings and evaluate the effectiveness of previously implemented plans.
  • Conduct home and nursing home visits as needed and facilitate the continuum of care for residents.
  • Maintain accurate documentation of all services provided according to professional standards.
  • Participate in quality assessment activities including record reviews and outcome measurement.
  • Provide education and training to participants, caregivers, and staff in relevant areas.
  • Act as a resource to program staff and participate in defining nursing standards.
  • Enhance professional growth through participation in research activities and educational programs.

Requirements

  • Master's degree required.
  • Graduate of an accredited Nurse Practitioner program.
  • Licensed by the Commonwealth of Massachusetts Board of Registration in Nursing.
  • Valid driver's license and clean driving record required.
  • National Board Certification as a Geriatric, Adult, or Family Nurse Practitioner.
  • One to three years of Nurse Practitioner experience preferred, with at least one year in geriatric care.
  • Demonstrates broad knowledge of medical conditions and appropriate interventions.
  • Ability to establish and maintain effective working relationships with program participants, family caregivers, medical staff, and program staff.

  • Interest in working with community-based physicians.
