About the position
At Signify Health, part of CVS Health®, we are dedicated to building trusted relationships to enhance the health of individuals. We are currently seeking board-certified nurse practitioner contractors to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. This role allows you to work flexible hours while connecting with patients one-on-one and helping them re-engage with their health. As a Nurse Practitioner, you will conduct in-home visits that focus on a person's overall health and well-being, identifying potential barriers to care. In this position, you can earn between $2,500 to $4,000 each week, with an average of 6-9 visits daily at a base rate of $80 per completed assessment. Additionally, you have the opportunity to increase your per-visit rate by up to 135% or 160%, depending on the counties you work in, through our Market Multiplier program. You can also earn extra compensation via our pay-per-test model for visits where the health plan member qualifies for additional diagnostic and preventive tests. Shifts are available from Monday to Sunday, with no nights or weekends required. You can choose to work on a short-term or long-term basis, with pre-scheduled visits in-home with health plan members. We provide support for travel, including airfare, hotel, and rental car, depending on eligibility. Malpractice coverage is also included. This role is designed for independent contractors with a 1099 status, allowing for a flexible schedule that accommodates your personal and clinical commitments. During the visits, you will perform a medication and medical history review, as well as a physical evaluation in the health plan member's home. It is important to note that our Nurse Practitioners do not prescribe medicine, order lab tests, or alter existing treatment regimens. Requirements
- Valid RN License
- Certified Family Nurse Practitioner certification
- Master's degree in nursing or related field
- Experience in primary care
- Texas Nurse Practitioner License (preferred)
- Willingness to travel 50% (preferred)
