We are seeking a full-time Nurse Practitioner for a home health primary care opportunity in Queens, NY. Responsibilities
- Provide primary care to patients in their homes.
- Visit 4-6 patients each day to deliver care and support.
- Maintain high-quality standards based on quality metrics.
- Collaborate with healthcare teams to ensure comprehensive patient care.
Requirements
- Valid RN License.
- Certified Family Nurse Practitioner certification.
- 1+ year of experience as a Nurse Practitioner.
- 1+ year of home health experience as RN or APN.
- Active DEA license.
