About the position

The Nurse Practitioner in Hospice Home Care provides advanced nursing clinical care to patients enrolled in the VNS Hospice Care program. This role is essential in ensuring that hospice-eligible patients receive appropriate care in alignment with VNS Health's philosophy and nursing standards. The Nurse Practitioner works under general supervision and is responsible for both in-home and telephonic patient assessments, treatment planning, and collaboration with a multidisciplinary team.

Responsibilities

• Provides advanced nursing care and treatment to hospice patients in both home and via telephonic visits, including assessment, diagnosis, prescribing medication, and ordering tests.
• Performs in-person pre-admission face-to-face (F2F) visits for certification of terminal illness and hospice admission.
• Completes certification of death.
• Documents patient care services in accordance with VNS Health standards.
• Collaborates with patients, families, primary care physicians, and other team members for assessment and care planning.
• Participates in the on-call NP coverage rotation after hours and weekends for in-person pre-admission F2F visits.
• Travels to multiple regions visiting patients and families.
• Prepares and maintains accurate patient records, charts, and documents to support sound medical practice and reimbursement for services provided.
• Participates in special projects and performs other duties as assigned.

Requirements

• Licensed and currently registered to practice as Nurse Practitioner in New York State.
• DEA license required.
• Master's Degree in Nursing required.
• Minimum of three years clinical nursing experience required.
• Experience as a Nurse Practitioner preferred.
