About the position
Hearth Hospice is seeking dedicated PRN Hospice Nurse Practitioners to provide exceptional patient care in the Fort Oglethorpe area. This role involves evaluating health histories, diagnosing medical conditions, and developing personalized care plans while collaborating with patients, families, and healthcare practitioners to ensure a holistic approach to hospice care.
Responsibilities
- Deliver compassionate medical care, managing terminal illnesses and ensuring compliance with the state Nurse Practice Act under the supervision of a collaborating physician.
- Collaborate with the patient's attending physician and hospice medical director to assess eligibility for hospice care and manage ongoing care needs.
- Coordinate and oversee patient care in collaboration with the hospice interdisciplinary team and other healthcare practitioners.
- Engage in patient and family education and counseling, addressing medical, psychological, and spiritual concerns.
- Provide recommendations for patient eligibility for initial certification and recertification for hospice care to the Hospice Medical Director.
- Maintain up-to-date knowledge of Medicare/Medicaid and state and federal regulations for hospice services.
Requirements
- Master's degree in nursing with a minimum of one year of hospice or palliative care experience as an RN or NP preferred.
- Current unencumbered license in the State of Practice as a Registered Nurse and Nurse Practitioner.
- Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics).
- Excellent verbal and written communication skills, with the ability to articulate complex problems.
- Computer proficiency in typing and various programs, including EMR.
Nice-to-haves
