About the position

The Nurse Practitioner (NP) in Hospice provides specialized medical care to patients with terminal illnesses, focusing on palliative care and health management. This role involves evaluating patient health histories, diagnosing medical conditions, and developing individualized care plans under the supervision of a physician. The NP collaborates with patients, families, and healthcare teams to ensure high-quality hospice services, while also fulfilling regulatory requirements for hospice eligibility assessments.

Responsibilities

  • Provide medical care and manage terminal illnesses in compliance with the state Nurse Practice Act under physician supervision.
  • Complete history and physical examinations, evaluating patients' medical conditions and health histories.
  • Order and interpret results from laboratory and diagnostic tests or procedures, diagnosing health conditions and documenting findings.
  • Develop individualized plans of care, prescribe medications and treatments, and coordinate hospice care services.
  • Complete hospice face-to-face visit requirements to assess eligibility based on hospice conditions of participation.
  • Provide clinical information to the hospice medical director for eligibility interpretation for recertification.
  • Make recommendations regarding patient eligibility for certification and recertification for hospice care.
  • Review clinical information and coordinate patient care in consultation with the hospice interdisciplinary team and other healthcare practitioners.
  • Consult with the collaborating physician when the patient's plan of care is outside standardized practice and protocols.

Requirements

  • Graduation from an accredited School of Nursing and accredited Nurse Practitioner Program.
  • Master's degree with a minimum of one (1) year Nurse Practitioner experience and a minimum of one (1) year hospice or palliative care experience.
  • Current Nurse Practitioner license in California.
  • Prolonged or considerable walking or standing.
  • Ability to lift, position, and/or transfer patients.
  • Ability to lift supplies and equipment.
  • Considerable reaching, stooping, bending, kneeling, and/or crouching.
  • Visual acuity and hearing to perform required nursing skills.
  • Must be a licensed driver with an automobile that is insured and in good working order.
