About the position
DaVita is seeking a Full-Time Nurse Practitioner (NP) to join their innovative team in the Minneapolis, MN area. This role is primarily remote, allowing the NP to work from home while also visiting DaVita clinics in the Twin Cities. The position emphasizes patient-centered care, focusing on quality and coordination rather than volume. The NP will collaborate with healthcare teams to provide comprehensive care for patients, particularly those with chronic kidney disease and end-stage renal disease.
Responsibilities
- Coordinate and facilitate quality, cost-effective care for patients.
- Work autonomously and in collaboration with healthcare teams.
- Provide coordination of care between patients, families, physicians, and care teams.
- Conduct clinical assessments, diagnoses, and develop care plans for chronic patients.
- Document and evaluate patient care effectively.
Requirements
- Current license to practice in the state of Minnesota.
- Current Drug Enforcement Administration (DEA) license required or eligible to obtain within 90 days of hire.
- Current Cardiopulmonary Resuscitation (CPR) or Basic Life Support (BLS) certification.
- Minimum of two (2) years' experience as a Nurse Practitioner (NP) required.
- Demonstrated proficiency in clinical assessment, diagnosis, planning, implementation, documentation, and evaluation of chronic patients required.
- Intermediate computer skills and proficiency in MS Word, Excel & Outlook required.
Nice-to-haves
- Preferred experience as a Family Nurse Practitioner (FNP), Primary Care Nurse Practitioner (NP), or experience in Internal Medicine, Cardiology, Nephrology or Case Management.
- Spanish-speaking preferred, but not required.
