About the position

The Nurse Practitioner role at GHR Search involves providing compassionate home-based primary and palliative care within a community-focused setting. This position is integral to a nonprofit hospice organization that has been serving the community for over 40 years, emphasizing patient-centered care and collaboration with a dedicated medical team.

Responsibilities

• Provide home-based primary and palliative care to a growing patient panel of over 600.
• Collaborate with a dedicated team of medical physicians and Nurse Practitioners who specialize in hospice care, available 24/7 for support.
• Attend weekly team meetings in the office, with infrequent visits outside of these meetings.

Requirements

• Certified Nurse Practitioner with current NP experience in primary care, preferably with a geriatric patient population.
• Minimum of 3 years of experience as a Nurse Practitioner.
• DEA certification is necessary.
• Licensure in Minnesota and North Dakota is required, as the role involves crossing state lines.

Nice-to-haves

• Palliative care experience is a plus.
• Home-based care experience is highly preferred.
