About the position

The Nurse Practitioner (NP) or Physician Assistant at Nest Health is responsible for providing comprehensive primary, acute, chronic, and pediatric care to families in their homes. This role emphasizes a holistic approach to health, focusing on preventive care and health education while collaborating with a multidisciplinary team to ensure effective care delivery. The NP or PA will manage a panel of families, utilizing both in-home visits and virtual care modalities to enhance health outcomes.

Responsibilities

  • Manage a panel of families as the primary care provider to improve whole-family health through longitudinal primary and preventive care.
  • Practice with the support of a robust interdisciplinary team, including MDs, LCSWs, RNs, psychiatrists, dieticians, and medical assistants.
  • Utilize Nest Health's electronic health record and care management tools to collect data, document member interactions, and communicate with the team and community resources.
  • Attend daily team huddles and weekly case conferences to advise on physical health needs and establish health goals as part of member's NAP (Nest Action Plan).
  • Propose treatments for chronic and infectious diseases, prescribe and reconcile appropriate medications, and educate patients about their conditions and medications.
  • Order, administer, obtain, and record diagnostic tests.

Requirements

  • Completion of Physician Assistant or Nurse Practitioner accredited program and an active, unrestricted license to practice in Louisiana is required.
  • At least 2 years of experience providing clinical services to pediatric and adult patients with chronic medical and behavioral health conditions.
  • Demonstrated proficiency in clinical nursing skills such as blood drawing (venipuncture & phlebotomy), patient assessments, and care plan reinforcement.
  • Proven skills necessary for independent clinical decision-making and problem-solving.
  • Available to work a full-time, five-day work week and participate in a rotating call schedule that includes occasional evening and weekend days each month.
  • Maintains current CPR certification (BLS).
  • Experience working as the leader of an interdisciplinary care team preferred.
