About the position

The Nurse Practitioner position at Personic Health focuses on providing advanced wound care services in a mobile setting, primarily in patients' homes and assisted living facilities. The role requires a strong clinical background and the ability to perform various medical procedures independently, ensuring high-quality patient care. Comprehensive training is provided to equip team members with the necessary skills for effective wound management.

Responsibilities

  • Travel to facilities and patients' homes to provide wound care services.
  • Perform bedside wound care procedures and position patients appropriately for optimal care.
  • Conduct a wide range of medical procedures independently, including wound care practices, biopsies, debridement, and suturing.
  • Assess, diagnose, and develop treatment plans for patients with acute and chronic conditions.
  • Collaborate with physicians and healthcare professionals to provide comprehensive patient care.
  • Conduct patient education sessions regarding treatment plans and preventive care.
  • Maintain accurate medical records using electronic health record systems.
  • Participate in quality improvement initiatives and adhere to clinical practice guidelines.
  • Provide leadership and mentorship to nursing staff and other healthcare providers.
  • Stay informed about the latest advances in medical technology and treatments.

Requirements

  • Master's or Doctorate degree in Nursing from an accredited institution.
  • Current licensure as a Nurse Practitioner in Texas.
  • Certification as a Nurse Practitioner.
  • 1+ year of experience in a procedural heavy setting, with a focus on wound care management.
  • Proficiency in performing a variety of medical procedures with skill and confidence.
  • Excellent communication and interpersonal skills.
  • Strong critical thinking and problem-solving abilities.

Nice-to-haves

  • Experience in home healthcare settings.
  • Wound Care Certification (WCC).
  • New graduates with relevant training and clinical rotations are encouraged to apply.
