About the position
Optum is seeking a Nurse Practitioner to join the Home-based Medical Care team in Bartow County, GA. This role focuses on delivering home-based medical care to complex, chronic patients, particularly older adults, enhancing their quality of life by providing comprehensive medical services in their homes. The Nurse Practitioner will collaborate with an interdisciplinary team to develop and implement patient care plans, ensuring a seamless health journey for patients.
Responsibilities
- Practices in accordance with state laws and regulations governing advanced practice nursing.
- Establishes and maintains effective relationships with patients, caregivers, and interdisciplinary team members.
- Performs initial comprehensive assessments on newly enrolled patients and provides ongoing care.
- Develops and updates patient management care plans as needed.
- Completes follow-up and post-discharge assessments according to standard operating procedures.
- Prescribes appropriate diagnostics and interventions to avoid unnecessary acute admissions.
- Performs basic nursing procedures including IV placement, blood draws, injections, and wound care.
- Consults with clinical teams following patient transfers.
- Educates patients and caregivers on disease processes and wellness management.
- Provides counsel and education to staff as appropriate.
- Participates in ongoing meetings related to patient care and clinical excellence.
- Implements quality initiatives to ensure high standards of care.
- On-call for caseload during business hours and participates in after-hours on-call program.
- Keeps current on medical and nursing research and technology.
Requirements
- Completion of Nurse Practitioner accredited program.
- Current state RN license.
- Current NP license.
- Access to reliable transportation.
Nice-to-haves
- 2+ years clinical experience in primary care, home-based medical care, geriatrics, or palliative care.
- Knowledge of long-term care and community resources.
- Familiarity with CPT, ICD-10, and HCPC codes.
- Understanding of clinical standards of care.
- Awareness of NCQA requirements and CMS guidelines.
