About the position
The Nurse Practitioner (NP) position at HarmonyCares focuses on conducting Health Risk Assessments for patients in their homes. This role is essential in providing personalized, quality-based healthcare to individuals who have difficulty accessing traditional care. The NP will perform assessments to identify health conditions and needs, educate patients and families, and provide transitional care to prevent hospital readmissions.
Responsibilities
- Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions.
- Determine all physical, mental, and social needs present at the time of the visit.
- Educate and counsel patients and families based on assessment results, including recommendations.
- Perform follow-up visits in transitional care.
Requirements
- Current RN and NP licensure in the state of practice.
- Current CPR certification.
- Must be enrolled in both Medicare & Medicaid.
