About the position

The Nurse Practitioner (NP) position at HarmonyCares involves conducting in-home health risk assessments and transitional care for patients recently discharged from hospitals. The role focuses on providing personalized, quality healthcare to patients who have difficulty accessing care, ensuring they receive the necessary support to manage their health effectively.

Responsibilities

• Conduct comprehensive in-home health risk assessments to identify all active and chronic disease conditions.
• Determine all physical, mental, and social needs present at the time of the visit.
• Educate and counsel patients and families based on assessment results, including recommendations.
• Perform follow-up visits in transitional care.

Requirements

• Current RN and NP licensure in the state of practice.
• Current CPR certification.
• Must be enrolled in both Medicare & Medicaid.
