About the position

The Advanced Care Provider (Physician Assistant or Nurse Practitioner) role at Sollis Health involves providing concierge-level medical care in a virtual environment. The position focuses on triage and aftercare services, ensuring members receive timely and appropriate medical attention through telehealth consultations and follow-ups. This hybrid role requires flexibility to work both virtually and onsite as needed, emphasizing high-quality patient care and customer service.

Responsibilities

  • Triage incoming patients' needs, assess concerns, and determine the best course of action for care delivery.
  • Review medical inquiries from patients via email, text, or phone, and respond in a timely manner.
  • Perform telehealth/video consultations or in-person evaluations, including prescribing medications and ordering labs and imaging as needed.
  • Follow up with patients after visits to assess their condition and determine if additional care is required.
  • Document clinical evaluations and treatment plans in the EHR system.
  • Assist patients with hospital admissions and transfers as needed.
  • Coordinate follow-up care with patients' primary care providers or specialists.
  • Provide concierge-level care and customer service aligned with Sollis Health's mission and values.

Requirements

  • Bachelor's Degree required.
  • Graduate of an accredited PA or NP program required.
  • 3+ years of experience as a Physician Assistant or Nurse Practitioner in emergency medicine, urgent care, case management, or critical care.
  • DEA license required.
  • NCCPA or Family Nurse Practitioner certification required.
  • BLS, ACLS, PALS certification required.
  • Pediatric and adult care experience.
