About the position

The Nurse Practitioner or Physician Assistant at Central City Community Health Center is responsible for providing comprehensive healthcare services in family medicine, pediatrics, or obstetrics. This role involves working collaboratively within an interdisciplinary team to deliver patient care, manage chronic conditions, and support primary care providers. The position emphasizes quality health services to underserved populations in a culturally sensitive manner.

Responsibilities

• Elicit patient medical and social history and interact with patients regarding their medical care.
• Perform a review of systems and initiate lab and radiologic tests.
• Provide health teaching and counseling, and monitor chronic conditions.
• Review chart findings and provide services in accordance with established medical standards.
• Consult with other healthcare team members regarding patient needs and refer to appropriate resources.
• Supervise RN, LVN, Medical Assistant, Community Health Worker, and other staff providing direct patient care.
• Participate in case management and staff meetings.
• Provide patient and community education classes as requested.
• Assist in the training of clinical and volunteer personnel as assigned.
• Assist in data collection for evaluation and research purposes.
• Complete reports and documentation, including billing and medical records.
• Participate in the CCCHC clinical committee and after-hour/weekend call schedule.
• Demonstrate CCCHC's principle of providing quality health services to underserved populations.

Requirements

• Graduation from an accredited Nursing program for advanced practice required.
• Current CA licensed as a Nurse Practitioner required.
• A minimum of 1-year experience preferred.
• Bilingual in English/Spanish preferred.
