About the position

The Nurse Practitioner in Palliative Medicine at Crouse Hospital plays a crucial role in a multi-disciplinary clinical team, focusing on providing comprehensive care to patients with serious or progressive illnesses. This position involves direct patient interaction, collaboration with healthcare providers, and facilitating goals of care through independent consultations and assessments. The NP will engage in advance care planning, manage complex symptoms, and support patients and families during hospital admissions and crises.

Responsibilities

  • Work directly with patients and their families/support network to identify and facilitate goals of care.
  • Collaborate with attending physicians, specialty healthcare providers, nursing staff, and ancillary staff.
  • Conduct independent consultations and patient assessments.
  • Provide education and supportive resources to patients and families as needed.
  • Engage in advance care planning conversations with patients and decision makers.
  • Complete associated MOLST or HCP forms as required.
  • Assess and make recommendations regarding complex pain or symptom management in collaboration with the Palliative Medicine Service Physician.
  • Determine a patient's capacity to make healthcare decisions.
  • Facilitate family meetings and referrals to Hospice and Francis House.
  • Implement comfort care orders.

Requirements

  • Currently licensed as a Registered Nurse in New York State.
  • Currently licensed as a Nurse Practitioner in Adult, Family Practice or Specialty area in New York State.
  • Graduate of an accredited Nurse Practitioner program.
  • Minimum of two (2) years' clinical experience as a nurse practitioner, preferably in medical/surgical inpatient care, critical care, pain management, oncology, or Hospice.
  • Basic Life Support (BLS) certification required at the time of hire or within two (2) weeks after hire.
  • Advanced Cardiac Life Support (ACLS) required at the time of hire or within two (2) weeks after hire.
  • Maintenance of AHA Stroke Certification is expected.
  • Good communication skills and a desire to learn.
  • Ability and willingness to work collaboratively with other healthcare providers and as part of a multi-disciplinary team.

Nice-to-haves

  • Advanced Practice Certification in Hospice and Palliative Medicine (HPM) is preferred, but not required at time of initial employment.
  • Agreement to work towards attainment of Advanced Practice Certification in the subspecialty of HPM.
