About the position

The Hospital Medicine Nurse Practitioner/Physician Assistant (APP) at Mary Free Bed Rehabilitation is responsible for delivering medical care to hospitalized patients in collaboration with the Hospitalist physician. The role focuses on providing acute care to complex medical patients, coordinating discharge planning, and facilitating the transfer of care back to primary providers. The NP/PA will also consult and make referrals to other healthcare team members as necessary.

Responsibilities

  • Provides patient care, treatment, and services within the scope of their license, certification or registration.
  • Conducts comprehensive bedside assessments and writes orders/prescriptions for medications and diagnostic studies.
  • Coordinates scheduling of new patients, diagnostic evaluations, follow-up exams, and referrals to home care and community agencies.
  • Develops and maintains effective working relationships with referral sources and healthcare staff.
  • Facilitates professional communication with hospital employees and consulting physicians.
  • Demonstrates excellent customer service standards and behaviors.
  • Encourages teamwork and direct communication with co-workers.
  • Monitors customer satisfaction and facilitates problem-solving with patients and families.
  • Assists with daily patient rounding and medical care within approved protocols.
  • Completes required regulatory documentation in collaboration with the responsible physician.
  • Assists with discharge planning and writes prescriptions according to approved protocols.
  • Completes discharge summaries in collaboration with the attending physician.
  • Ensures compliance with state and federal regulations and organizational policies.
  • Participates in program development and service recovery as needed.

Requirements

  • Certified and licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) by the State of Michigan.
  • Preferable to have previous experience as NP or PA in Internal Medicine.
  • BLS, ACLS, and PALS certifications required.
  • Strong time management and organizational skills.
  • Effective written and verbal communication skills.
  • Ability to respond to a highly variable workload and maintain flexibility in work hours.
  • Excellent computer skills, including proficiency in Microsoft Office and Electronic Medical Records.
  • Reliable transportation to access area hospitals on a daily basis.
