About the position

The Hospital Medicine Nurse Practitioner/Physician Assistant (APP) at Mary Free Bed Rehabilitation Hospital plays a crucial role in delivering medical care to hospitalized patients in collaboration with the Hospitalist physician. This position focuses on providing acute hospital care to complex medical patients, ensuring that they receive comprehensive and coordinated care throughout their hospital stay. The Nurse Practitioner (NP) will work closely with an interdisciplinary care team to facilitate discharge planning and ensure a smooth transfer of care back to the patient's primary provider. This involves consulting and making referrals to physicians and other healthcare team members as appropriate, thereby enhancing the overall patient care experience. In this role, the NP/PA is responsible for providing patient care, treatment, and services within the scope of their license, certification, or registration, adhering to all relevant laws and regulations. They will conduct comprehensive bedside assessments, write orders and prescriptions for medications, and coordinate various diagnostic evaluations and follow-up exams. The NP/PA will also be tasked with developing and maintaining effective working relationships with referral sources, hospital employees, consulting physicians, and staff, ensuring that communication remains professional and effective. The position requires a strong commitment to customer service, as the NP/PA will monitor customer satisfaction and facilitate problem-solving with patients, families, and referral sources. They will assist with daily patient rounding, discharge planning, and regulatory documentation, ensuring compliance with state and federal regulations as well as organizational policies. The NP/PA will also participate in program development and service recovery initiatives as needed, contributing to the continuous improvement of patient care services.

Responsibilities

  • Provides patient care, treatment, and services within the scope of their license, certification or registration and as required by law and regulation.
  • Conducts comprehensive bedside assessments and writes orders/prescriptions for medications, laboratory studies, and consultations.
  • Coordinates the scheduling of new patients, diagnostic evaluations, follow-up exams, and referrals to home care and social services.
  • Develops and maintains effective working relationships with referral sources, hospital employees, and consulting physicians.
  • Facilitates professional communication with employees of the referring hospital and the medical group.
  • Demonstrates excellent customer service standards and behaviors.
  • Encourages, coaches, and monitors teamwork and direct communication with co-workers.
  • Handles confidential information discreetly and sensitively.
  • Monitors customer satisfaction and facilitates problem-solving with the Internist, patients, families, and referral sources.
  • Assists with daily patient rounding and medical care within approved protocols as directed by the Internist.
  • Writes medication orders within approved protocols or in collaboration with the responsible physician.
  • Completes required regulatory documentation in accordance with policies, in collaboration with the responsible physician.
  • Assists with discharge planning for patients and completes discharge orders and prescriptions according to approved protocols.
  • Completes the discharge summary in collaboration with the attending physician.
  • Ensures services are provided in accordance with regulations, organizational policy, and accreditation requirements.
  • Performs individual assignments as directed by management.
  • Provides information to senior management on implications of policies and procedures being formulated and recommends actions.
  • Participates in program development and service recovery as needed.

Requirements

  • Certified and licensed as a Nurse Practitioner (NP) or Physician Assistant (PA) by the State of Michigan.
  • Preferable to have previous experience as NP or PA in Internal Medicine.
  • BLS, ACLS, and PALS certifications required.
  • Strong time management and organizational skills.
  • Effective written and verbal communication skills.
  • Ability to respond to a highly variable workload including flexibility of work hours.
  • Ability to maintain effective communication with multiple physicians and referral hospital employees.
  • Excellent computer skills, specifically in Microsoft Office Products and Electronic Medical Records.
  • Reliable transportation and ability to access area hospitals on a daily basis.
