About the position

La Clnica de La Raza is a community-based health center committed to providing culturally appropriate, high-quality, and accessible healthcare to the diverse communities of the San Francisco Bay Area. We have spent over 40 years advocating for and creating a health home for the many that have been denied access to care. As healthcare activists, we are dedicated to making sure individuals who do not have health coverage get the same level of quality healthcare as those who have it. From our genesis as a community health center in 1971, we understood that we cannot heal our communities without also addressing the economic and social factors that affect health. Many times, this requires that we go outside and provide services in other settings to build the bridge that links individuals to our health center for preventive and primary care. Over 90,000 individuals come to La Clnica because it is a welcoming place that addresses the whole person, coordinating and connecting them to a broad network of services to improve and maintain their health and well-being. While we are still known for our activism and spirit of social justice, we are also proud to have grown into a sophisticated provider of primary health care services with 35 sites across Alameda, Contra Costa, and Solano counties. At La Clnica, you will be able to provide primary care in an outpatient setting, allowing you to practice comprehensive medical care. You will deliver care to a resilient patient population that is ethnically diverse and medically complex, with the aim of achieving optimal health outcomes for every patient. We attract and retain engaged staff by supporting providers in their pursuits of a meaningful role and offering opportunities in mentorship, quality initiatives, and preceptorships. La Clnica is committed to continued provider education; we host a Nurse Practitioner incubator program to prepare new graduates for their roles. As a NPPA, you will be surrounded by a collaborative care team that is invested in your success and shares ownership of tasks to help improve your efficiency as a provider.

Responsibilities

  • Provide medical triage of patients
  • Medical care management and patient education and counseling
  • Make referrals to specialty providers and follow up to ensure that patients' bio-psychosocial needs are addressed
  • Deliver evidenced-based care that supports our aim for improved health outcomes, exceptional patient experience, and efficient use of resources
  • Perform routine acute, urgent, preventive, curative, and rehabilitative medical care to patients and determine appropriate regimen
  • Serve as a member of the cross-functional team of providers engaged in routine and preventive primary care, urgent care, and telephone triage
  • Prescribe medications, treatments, or therapies and order medical diagnostic or clinical tests
  • Deliver well and chronic disease care with cultural humility to a diverse patient population of all ages
  • Have flexibility in meeting patients' needs and participating in evening and Saturday rotation clinics
  • Develop a positive rapport with patients and families to foster the provider-patient relationship
  • Conduct in-home assessment of patients as appropriate
  • Serve as a member of the cross-functional team of providers consulting on the assessment, management, and treatment of complicated cases
  • Chart review, co-signing, and case conferences
  • Consult with physicians on the management, assessment, and treatment of specific cases as needed and per protocol
  • Suggest improvements for clinic flow, patient scheduling, and service delivery, and collaborate professionally with interdisciplinary team members
  • Supervise medical assistants by providing patient consultation as needed

Requirements

  • Knowledge and experience with Electronic Health Record programs such as NextGen or Epic
  • High degree of interpersonal competence with both staff and patients and thorough knowledge of evidence-based care
  • Cultural competence working in diverse, low-income communities
  • Familiarity with Electronic Panel Management
  • Ability to deal courteously and effectively with complex patient profiles and maintain good working relationships with other staff
  • Must have the physical stamina to function effectively in a position that involves heavy lifting (50lbs)
  • Ability to work courteously and effectively as a team member to support the quality and efficiency standards established for the clinic
  • Ability and willingness to work under pressure and as part of an interdisciplinary team
  • Demonstrate flexibility and ability to accept changes gracefully
  • Ability to work across cultures and demonstrate support of diversity, equity, and inclusion
  • Ability to keep accurate and clear records of patient information
  • Top-notch time management skills and evidence-based approaches to medical care
  • English/Spanish proficiency preferred but not required

Nice-to-haves

  • Current BLS Basic Life Support Certificate
  • Board certification
  • Valid California license
  • Current license from the California Board of Registered Nursing or California Physician Assistant Committee
  • National certification from the American Academy of Nurse Practitioners or other national certifying bodies
  • Certificate of completion of a Nurse Practitioner Program with a minimum of 2 years clinical experience
  • Graduation from an approved program of instruction in primary health care as attested by the American Medical Association
  • Passing a certification examination administered by the Bureau of Medical Quality Assurance
  • Minimum of two years of full-time clinical experience after clinical practicum with emphasis in the areas of family medicine or geriatrics
