About the position

The Nurse Practitioner/Physician Assistant position at Alignment Health focuses on providing exceptional care to seniors through the Alignment Jump Start Assessment (JSA) program. This role involves working within an interdisciplinary team to assess, develop, and coordinate care for patients with complex and chronic needs, ensuring compliance with healthcare regulations and promoting optimal health outcomes.

Responsibilities

  • Conduct in-home assessments on health plan members.
  • Obtain comprehensive history, physical exam, medication review, and appropriate cognitive/depression/safety screenings during assessments.
  • Engage in end-of-life care discussions as necessary.
  • Collect laboratory specimens in the home setting when appropriate.
  • Identify diagnoses for active medical management and care management of patients, focusing on chronic disease management.
  • Communicate assessment findings to inform the primary care physician of potential gaps in care.
  • Provide education to patients, families, and caretakers, emphasizing close monitoring and follow-up of patient needs.
  • Utilize knowledge of community resources for appropriate referrals.
  • Comply with all HIPAA regulations and maintain the security of protected health information.

Requirements

  • Master's degree from an accredited Nurse Practitioner or Physician Assistant program.
  • Preferred one year of prior clinical or home care experience.
  • Previous experience with electronic medical records (EMR) preferred.
  • Active California state Nurse Practitioner or Physician Assistant license.
  • Active RN License and Furnishing number (for Nurse Practitioners only, must be obtained upon hire).
  • Active Nurse Practitioner Board Certification or Physician Assistant Certification.
  • NPI Number, DEA, and valid BLS certification required.
  • Valid California driver license and current automobile insurance.
  • Experience in care of older adult (geriatric) patients preferred.
  • Excellent administrative, organizational, and communication skills.

Nice-to-haves

  • Experience in home care settings.
  • Familiarity with chronic disease management.
