About the position

The Nurse Practitioner/Physician Assistant (NP/PA) role at CCA Primary Care is crucial for providing comprehensive primary care to patients with complex health conditions. This position involves working closely with an interdisciplinary care team to deliver high-quality medical care in various settings, including clinics, home visits, and virtual consultations. The NP/PA will focus on chronic disease management, preventative care, and coordination of care, ensuring that patients receive timely and appropriate medical attention to improve their overall health outcomes.

Responsibilities

  • Provide clinical care to patients in clinic, through home visits, and utilizing telehealth technologies.
  • Facilitate and deliver preventative care to patients according to clinical guidelines.
  • Identify and initiate plans to meet quality, STAR, and HEDIS measures.
  • Conduct Annual Visits with patients, taking detailed histories and performing physical examinations.
  • Perform episodic urgent medical visits to provide timely care and avoid emergency room visits.
  • Prescribe medically necessary medications and manage medication-assisted treatment when appropriate.
  • Perform post-discharge visits to ensure continuity of care and medication reconciliation.
  • Independently make complex medical decisions and prioritize tasks effectively.
  • Ensure appropriate and timely documentation of patient visits and activities.
  • Liaison with internal care teams and external specialists as necessary.
  • Provide consultation and support to other care team members.
  • Participate in team case reviews and practice meetings.
  • Actively participate in self-evaluation and competency advancement activities.
  • Contribute to quality improvement efforts and strategic initiatives within the organization.

Requirements

  • Graduation from an accredited Physician Assistant or Nurse Practitioner program.
  • Current board certification for Physician Assistants or Nurse Practitioners.
  • Completion of training related to prescriptive practice as per Massachusetts law.
  • Current CPR or Basic Life Support (BLS) Certification.
  • Evidence of a valid license to practice as a PA or NP in Massachusetts.
  • Current Massachusetts Controlled Substances License and DEA Controlled Substances License.

Nice-to-haves

  • 2+ years of relevant experience in a clinical setting.
  • Bilingual skills preferred.
