About the position

The Nurse Practitioner or Physician Assistant for the Stroke Service at Jersey Shore UMC is responsible for providing patient-centered care through a coordinated care model. This role involves leadership, direct clinical practice, consultation, and collaboration with healthcare teams to assess, plan, and implement care for patients with health and safety needs.

Responsibilities

  • Provides medical assessments, treatments, and clearances for HMH applicants and external customers according to regulatory standards.
  • Performs clinical care professionally, serving as a customer advocate and role model.
  • Ensures clinical practice aligns with NJ Board of Medical Examiners regulations and HMH policies.
  • Develops and implements appropriate plans of care for patients.
  • Facilitates patient teaching and support programs for patients and employers.
  • Evaluates current protocols and standards of care to translate best practices into clinical practice.
  • Supports the development of evidence-based practice protocols and standards of care.
  • Applies evidence and value-based practices to achieve positive patient outcomes.
  • Provides medication safely and effectively to promote recovery from on-the-job injuries or illnesses.
  • Prescribes medications and recommends over-the-counter medications as appropriate.
  • Initiates laboratory tests, immunizations, and diagnostic tests as per regulations.
  • Demonstrates competent clinical skills with treatment plans using effective screening tests.
  • Collaborates with physicians and consults with other healthcare team members effectively.
  • Demonstrates effective interdisciplinary communication skills to achieve results.
  • Provides accurate and organized documentation of findings and education.
  • Participates in continuing education and maintains documentation of competencies.
  • Provides culturally diverse and educationally appropriate patient education.
  • Serves as a clinical resource to nursing staff and multidisciplinary team members.
  • Participates in policy/procedure reviews to update with current evidence-based practices.
  • Acts as a resource for staff to address clinical and process improvement opportunities.
  • Promotes a collaborative work environment supporting organizational values.
  • Seeks opportunities to bring new clinical knowledge into practice.
  • Identifies ethical care issues and coordinates meetings to resolve them.
  • Floats to various clinic locations as needed.
  • Performs other duties and projects as assigned.
  • Adheres to HMH Organizational competencies and standards of behavior.

Requirements

  • Graduate of an NLN/AACN accredited program in nursing.
  • Masters Degree in nursing or related field.
  • Maintains clinical competencies through continuing education and clinical practice as mandated by the Board of Nursing and national certification body.
  • Experience in a relevant patient care setting as regulations dictate.
