About the position
The Nurse Practitioner position at Matrix Medical Network involves conducting home risk assessments for at-risk health plan members. This role is designed to provide comprehensive health evaluations that improve care quality and close care gaps. The position offers flexible scheduling and the opportunity to make a significant impact on the health and well-being of individuals in the community.
Responsibilities
- Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and documentation according to Matrix guidelines.
- Work collaboratively with physicians, case managers, social workers, family members, and other medical personnel.
- Collaborate with Primary Care Physicians on patient education and follow-up care.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
- Master's Degree required or equivalent experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it.
- Board certified by AANP, ANCC, or AACCN in a Matrix approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with health/medical record software.
- Excellent verbal and written communication skills.
Nice-to-haves
- Experience in home health or community health settings.
- Familiarity with telehealth services.
