About the position
The Nurse Practitioner (PRN/Part Time) at Matrix Medical Network is responsible for conducting comprehensive health assessments for at-risk health plan members in various settings, including home visits and telehealth. This role emphasizes collaboration with healthcare teams to improve patient care and outcomes, while providing flexible scheduling options to accommodate the needs of both the provider and the patients.
Responsibilities
- Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and documentation according to Matrix guidelines.
- Work collaboratively with physicians, case managers, social workers, family members, and other medical personnel.
- Collaborate with Primary Care Physicians on patient education and follow-up care.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
- Master's Degree required or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or the ability to obtain it.
- Board certified by the AANP, ANCC, or the AACCN in a Matrix approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent change.
