About the position
The Nurse Practitioner position at Matrix Medical Network involves conducting comprehensive health assessments for at-risk individuals in their homes or community settings. The role focuses on evaluating patients' health, medical history, and social environments to improve care quality and close care gaps. This part-time, PRN role offers flexible scheduling and the opportunity to make a significant impact on patients' health outcomes.
Responsibilities
- Conduct Adult/Geriatric Assessments, including medical history, diagnosis, treatment, health education, and documentation according to Matrix guidelines.
- Work collaboratively with physicians, case managers, social workers, family members, and other medical personnel.
- Collaborate with Primary Care Physicians on patient education and follow-up care.
- Provide services in various venues, including home visits, skilled facility visits, and telehealth visits.
Requirements
- Master's Degree required or equivalent experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, including prescription authority or ability to obtain it.
- Board certified by AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with health/medical record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
- Comfortable and flexible with frequent changes.
